Athletics Paperwork
ALL student-athletes should complete the UIL mandated forms EVERY year.
If you are a 7th grader, Freshmen or Junior, you will also need an updated physical.
If you did not participate in athletics this past year or did not get a physical last year you MUST have a new physical on file no matter the grade.
Use the link below to access RankOne and fill out your UIL paperwork. You can continue as a guest or create your own account. You will still need fill out the PPE Form in person due to its medical content. The first page of the PPE Form needs to be filled out every year. The back of the PPE Form is what needs to be filled out by a doctor every other year.
*** If you are new to the district (move-in, transfer, St. Anthony's) you will not have a student ID to fill out the paperwork online. You will need to fill out the forms listed below. You can either print out the forms yourself, or you can pick up a packet from Miss. Kasey.
