Using this site, parents can access current information about state and local requirements regarding their child, as well as information about specific health concerns and conditions. SB 12 Information: Services Provided by CISD Health Service Department
  • Administer basic care and first aid for minor illnesses and injuries (e.g., headaches, stomach aches, sore throats, fever, cuts, sprains, bruises, and minor burns).
  • Administer basic treatments such as ice administration, eye/eyewear care, school provided topical ointments. (e.g., salt, saline eye wash, vaseline, sting relief,  aloe vera)
  • Provide care as ordered by my child’s physician or care provider for their chronic health condition. (e.g., asthma, seizures,diabetes)
  • Perform as needed screenings based on assessment needs. (e.g., lice (parasitic), height and weight (growth), blood pressure, and dental checks).
  • Administration of Parent Provided OTC and Prescription Medications: Will only be administered with a signed Medication Permission form available at the campus or district website. 
  • State Mandated Screenings: Will be performed unless an opt-out notice is received before the screening. (If you choose to opt-out, you must take your child to your healthcare provider and obtain the screening at your cost and provide the results to the school nurse as mandated within 60 days, provide a signed, notarized religious exemption form, or submit proof of an examination within the last year).

Right to Withhold Consent: I understand that I have the right to withhold consent for or decline any health-related service(s) provided by the school at any time by notifying the school, in writing, and I understand that withholding consent is only effective after it is received and logged by the district nurse or authorized designee. I also understand that I may change my mind at any time by submitting a request in writing to the district nurse.

New guidelines for medication administration!
A "school nurse should have a signed order (from MD, DO, APRN, PA, dentist, or podiatrist) before administering any medication (prescription or OTC) to meet the standards of the Nursing Practice Act. This includes oral and topical medication (such as cortisone or other ‘itch’ creams, antibiotic ointment, Orajel, cream, or gel with lidocaine, sting relief pads, etc)." - Texas Board of Nursing 

Columbus ISD follows the guidance of national and local health officials to help protect our students and staff from all communicable diseases.  The District is aware that the Centers for Disease Control and Prevention is monitoring an outbreak of a respiratory illness caused by the coronavirus. Please be assured Columbus ISD school health officials closely monitor all developments and are in communication with public health agencies. CISD will adhere to all guidance provided by our school health officials in regard to protecting students and staff against the transmission of communicable diseases, including guidance regarding the coronavirus.
